Before joining, it's important that you read the Equip Super MyFuture Product Disclosure Statement, and refer to the Target Market Determination (TMD) for the product, prior to completing the form.
Note:
If your employer already uses Equip Super as their default fund, please do not use the online application form, as you’ll miss out on any default insurance you may be eligible for. Ask your payroll department to set you up in Equip Super, and your account will become active once your employer starts making contributions for you.
